KYT_0488bAs an experienced consultant in both cardiology and medicine, Jeremy provides outpatient and inpatient management of a wide range of cardiac and medical conditions. Within the NHS he delivers cardiology inpatient services at Calderdale Royal Hospital and Huddersfield Royal Infirmary. He runs a sub-specialist complex angioplasty clinic assessing patients for Chronic Total Occlusion Angioplasty to coronary vessels on Friday mornings. These are the most challenging cases that Jeremy has taken an interest in treating. Jeremy is a high volume angiography and angioplasty operator supporting both elective and acutely unwell patients at Calderdale and Huddersfield, whilst supporting 24/7 regional emergency angioplasty services at Leeds General Infirmary catering for wider patients within West Yorkshire.

In the private sector Jeremy holds outpatient clinics at Spire Elland Hospital  and Yorkshire Clinic. He has admitting rights for inpatients at Spire Elland Hospital, Yorkshire Clinic and Nuffield Leeds Hospital where interventional cardiac procedures can be performed if required. He is a registered provider with many established medical insurance companies including Vitality, WPA and AVIVA. Many patients choose to self pay whether this be for ongoing care or just for an initial consultation. This offers them added convenience, rapid access to appointments, a consultant opinion along with continuity of care, seeing the same doctor on each occasion.

Patients present with very varied underlying medical problems, from the very straightforward to the very complex. Jeremy is happy to undertake more routine cardiac health screening to identify and modify future cardiovascular risk. Many patients have a range of additional non cardiac conditions and Jeremy enjoys taking a holistic approach to their care. This is of particular relevance increasingly in older patients who may be taking a range of different medications. Where conditions require additional specialist input then Jeremy has developed a wide network of colleagues to whom he can refer when needed.

From a social and vocational perspective, Jeremy is able to offer cardiac assessments for individuals undertaking increased levels of exercise, whether this be more mature people considering gym membership or individuals performing competitive sport. He is able to complete cardiac DVLA assessments which are often required for individuals wishing to gain Group 1 and 2 licenses including HGV and taxi drivers.

Competitions and Markets Authority

All private doctors must provide you with appropriate information related to pricing. This allows you to make an informed decision about your care and its cost. We feel that pricing transparency is central to the services delivered, giving patients confidence at each and every stage of their investigation and management. Patients are seen in clinics at the Spire Elland and Yorkshire Clinic. At the time of booking an appointment both hospitals have committed to sending patients written information related to outpatient consultation fees in line with recommendations from the Competition and Markets Authority. This is in addition to verbal clarification whilst on the telephone to the appointments service. We understand that many patients with pressing symptoms and concerns will want to be seen at very short notice. Dr Butts will also clarify the information prior to each outpatient consultation ensuring that you do not need to worry about unexpected consultation costs. Consultation costs can vary between individual insurance providers but no new patient consultation either insured or self pay will exceed £230 and no follow up will exceed £150. This excludes the cost of any further tests that may or may not be required depending on individual circumstance. 

Where further investigation or treatments are agreed during a consultation Dr Butts will whenever possible let patients know the cost at the time of the consultation. This will be followed up with written confirmation of the intended tests or treatments, the reason for them being performed and the location of the tests. It will include relevant information on pricing or contact details of the hospitals delivering tests to make it easy for patients to confirm costs before proceeding. If you have private medical insurance, please contact your insurer before your consultation, to check the terms of your policy, particularly the type and level of outpatient cover that you have including any reimbursement limits on individual consultation fees.

Outpatient Appointment

Thank you for choosing to see me at Spire Elland Hospital for your outpatient consultation. You should have already received information by post but this is an opportunity to confirm details prior to your appointment. This letter sets out some important information that I am required by law to provide to you. This is for your information only and is not a bill. As this includes information about my charges, if you do not have private medical insurance but someone else will be paying your bill, you may wish to pass a copy of this letter to them. Please note that even if someone else is paying your bill or you have private medical insurance, you are responsible for paying any charges that they do not pay.

My fee for an initial consultation will not exceed £230 and my fee for any follow up consultation will not exceed £150. These estimates are correct a the date of this letter. I do not charge cancellation fees but would be very grateful if you could give as much notice as possible to allow other patients to be allocated appointments.

Following your consultation you may need certain tests (such as blood tests or imaging, for example and X ray, MRI or CT scan) to help me diagnose your condition. If the test is undertaken by the clinic or hospital and not by me then the fees for those tests will be determined by the clinic or hospital and charged to you or your private medical insurer separately.

If there are any additional fees that I will charge separately in relation to any of the tests that you have, then I will let you know what those may be. Otherwise you can assume that they are included in the hospital price of the test itself.

Private Medical Insurance

If you have private medical insurance, please contact your insurer before your consultation, to check the terms of your policy, particularly the type and level of outpatient cover that you have including any reimbursement limits on individual consultation fees.

Data Privacy Notice

We take your data privacy very seriously. This notice informs you how your data is stored and processed making the service transparent and clear. We hope this offers you confidence in the services we provide. 

What personal information is held about you and how do we use it?

We hold information including personal identifiers, addresses, contact details and insurance details for communication. This is in addition to an electronic copy of your medical letters and invoices that we generate for you. Additional information may include the results of investigations and communications from providers involved in your care. Much of this you provide us with, but additional details may come from third parties such as the hospitals in which you are seen, your general practitioner and others involved in your care. Holding this information is necessary for diagnosis, treatment and management of health services. It allows us to provide you a higher quality of service.

How long do we retain your personal information?

Our policy is to retain your personal and medical information for up to 30 years. We find it helpful for patients who need over coming years to continue their care after an interval. It allows us to continue your care without unnecessarily needing to collect all of your details once more and reduces the expense to you if you need to represent for a review. We reserve the right to delete your data earlier than 30 years if at any stage the company ceases to practice or at your request.

How will my personal information be shared?

All of your clinical letters are routinely copied to yourself, your general practitioner in addition to a copy letter given to the hospital in which you were seen. Letters may also be copied to other clinicians and hospitals involved in delivering your care. They are sent by standard first class post using Royal Mail. This means that you should always have access to your information even in the event that electronic data is subsequently erased. Your information may be shared with your insurance company to facilitate invoicing and assist them with processing your claims. We use a company called Healthcode who process electronic invoices for a majority of insurance providers. We do not undertake any direct marketing or share your information with third parties for marketing purposes.

How is my personal information kept secure?

We use a market leading cloud based Practice Management Software run by ClanWilliam Health. The system ensures a very high level of security and all data is strongly encrypted. Access to your data requires a number of confidential passwords in addition to an RSA physical encryption fob that changes its code every minute. This is a higher level of security than present for many banking accounts. The system allows us to access your data in multiple settings, allowing you to be seen in different clinics at your convenience. Your information is accessible by ourselves in hospitals where you undergo investigations enhancing safety and can be very helpful facilitating your NHS care. It allows your care to transition across boundaries without unnecessary delay. Our email provider is Confidesk a fully encrypted provider. The email account is only accessibly by ourselves and one of the most secure. As however email relies on password protection without a physical encryption fob we consider it less secure and ask that it is used only for administrative communications such as appointment requests. We ask that no clinical or sensitive information is sent via this route to protect your privacy. Dictation is undertaken digitally and fully encrypted to protect against data loss which could otherwise result from loss of tapes or physical devices.

What are my choices with my personal information?

You are able to ask us what information we hold about you and request that personal information is amended where incorrect. We will let you know when updated. If you object to your data being processed then we will stop processing it once no longer required for performance of contract or due to legal obligations. You can request that your personal information is erased once no longer needed for performance of contract or legal obligations.

Your rights to lodge a complaint with the ICO?

If you have concerns about the way your data is handled then you have the right to complain to the Information Commissioners Office (www.ico.org.uk)
